Payment and Insurance
Payment is expected at the time of service. We will submit charges to your insurance company and will verify all insurance benefits. This verification is not, however, a guarantee of payment by the insurance company. Please bring your insurance information and ID card to all visits. Arrange for appropriate referrals if required by your insurance plan. If alternate financial arrangements are needed, please speak with an office manager prior to your appointment.
